Applying for a new Mauritian passport
Getting the Application Form
You can obtain the Passport Application Form (PF136) in the following ways:
- Visit the Passport & Immigration Main Office in Port Louis or its Sub offices.
- Visit any Police Station in Mauritius.
- Visit the nearest Mauritius Embassy/Consulate Office if you are abroad.
- Download the forms from the Passport and Immigration Office website:
Filling out the Form
Before you start filling out the form, read the "NOTES" on the front page carefully.
Complete all the necessary sections of the form using CAPITAL LETTERS and a BLACK ink pen.
- If the applicant is a minor (under 18 years old), both parents (if applicable) must sign the form in the presence of a police officer at a police station or at the Passport and Immigration Office.
- If the parent(s) are abroad, they can provide consent in the presence of:
- A Police Officer at a police station.
- An Official from a Mauritius Embassy/Consulate.
- A Notary or Solicitor from the applicant's country of residence.
If it's not possible to certify parental consent as mentioned above:
- The parent(s) should sign in section 4A.
- The parent(s) should send a letter by registered post to the Passport and Immigration Office, authorising their child to be issued with a Mauritius Passport.
- The letter should include the exact names of the parent(s) and the applicant, matching the documents submitted.
- The parent(s)' signatures must match section 4A of the application form and the submitted documents.
- The identity and address of the individual for whom the passport is to be issued must be verified by the police in the area where he/she resides before submitting the application form.
- This section not applicable if the applicant is abroad.
- The specimen signature or right thumbprint of the applicant, which will appear on the passport, should be made using a bold black ink pen with a size of 0.6 mm, within the inner line of the box.
- It is mandatory for the applicant to sign or place their right thumbprint on the application form in section (8).
- For minors, either a parent or legal guardian should sign the declaration in section (8) of the application form.

Passport applications for citizens abroad
All applications must be submitted through the Mauritius High Commission, Embassy, or Consulate.
Documents required
Applicants should submit copies of the following documents, as applicable, along with the filled Application Form (PF 136). Original documents should be available for verification.
All required documents should be in English or French. Documents in other languages must be translated by a registered sworn translator, and if made abroad, the document should be apostilled, legalised, or authenticated.
Adults (18+ years)
- Filled and signed Application Form (PF 136).
- Birth Certificate.
- Marriage Certificate (if applicable).
- Mauritius National Identity Card.
- Certificate of Foreign Citizenship (if applicable).
- Certificate of Mauritius Citizenship (if acquired through Registration, Naturalisation, or Adoption, if applicable).
- Two recent identical passport-sized photos taken against a light background.
- If born abroad:
- Birth Certificate of Mauritian parent(s).
- Certificate of Foreign Citizenship or Residence Permit of Mauritian parent(s).
- Additional documents as applicable.
Minors (under 18 years)
- Filled and signed Application Form (PF 136) by the applicant and Parents/Guardian.
- Birth Certificate.
- Birth Certificate of parents.
- Mauritius National Identity Card of parents.
- Passport of parent(s) (Applicable to Non-Citizen of Mauritius only).
- Certificate of Foreign Citizenship or Residence Permit of parent(s) (if applicable).
- Certificate of Mauritius Citizenship of parent(s) (if applicable).
- Judge’s Order (if applicable).

Renewal of a Mauritius Passport
Adults (18+ years)
Documents required
- Filled and signed Application Form (PF 136A).
- Previous passport (+ photocopy of bio-data page only).
- Mauritius National Identity Card.
- Marriage Certificate (if applicable).
- Divorce Decree (if applicable).
- Certificate of Foreign Citizenship (if applicable).
- Two recent identical passport-sized photos taken against a light background.
- Additional documents as applicable.
Note: Married persons applying for a passport with 'Joint Name' will receive it in the surname at birth followed by the spouse's name (without a hyphen).
Minors (under 18 years)
- Filled and signed Application Form (PF 136A) by the applicant and parents/Guardian.
- Previous passport (+ photocopy of bio-data page only).
- Birth Certificate of the applicant.
- Mauritius National Identity Card of parents.
- Passport of parent(s) (Applicable to Non-Citizen of Mauritius only).
- Judge’s Order (if applicable).

Replacement of a Lost/Stolen/Damaged Valid Passport
Documents required
- Filled and signed Application Form (PF 136) by the applicant and parents/Guardian (if applicable).
- Birth Certificate of the applicant.
- Birth Certificate of parents (if applicable).
- Mauritius National Identity Card of the applicant.
- Mauritius National Identity Card of parents (if applicable).
- Marriage Certificate of the applicant (if applicable).
- Marriage Certificate of parents (if applicable).
- Divorce Decree of the applicant (if applicable).
- Certificate of Foreign Citizenship or Residence Permit Abroad of the applicant (if applicable).
- Certificate of Foreign Citizenship or Residence Permit of parent(s) (if applicable).
- Certificate of Mauritius Citizenship of parent(s) (if applicable).
- Certificate of Mauritius Citizenship (if acquired through Registration, Naturalisation, or Adoption, if applicable).
- Two recent identical passport-sized photos of the applicant taken against a light background.
- Certificate from Police in case of loss/stolen passport.
- A letter detailing how, where, and when the passport was lost/stolen/damaged.
- The original Police report for the lost passport (including reference, signature, and seal/stamp of the Police Station) or a report made by the Embassy or High Commission of Mauritius.
- Additional documents as applicable.
Where Mauritius High Commission / Embassy or Consulate is not available, application may be submitted through a close relative in Mauritius.
Documents required
In addition to the documents mentioned above:
- A letter from the applicant authorising a close relative in Mauritius to submit the application and collect the passport.
- For applicants under 18 years, consent of parents on the Application Form should be certified by:
- The Police Officer, including their name, signature, and the seal/stamp of the Police Station; or
- An Attorney-at-Law, Barrister, or Notary, with their name, signature, and the official seal/stamp of their Office.
